>>  James A. D'Orta, MD

Dr. D’Orta is a physician and businessman who was a Board Certified Specialist in Emergency Medicine and the Founder and President of International Medical Consulting, Inc. (IMC). IMC has provided medical support and consulting services to corporations and individuals over the past 10 years. Most recently, Dr. D’Orta served as an investor, medical advisor and board member to Minute Clinic, which was acquired by CVS, Inc., and now operates over 100 retail-based health clinics in 15 states. Prior to that position, he was the founder and CEO of a national physician service and medical device oversight provider, LifeLink MD, which was recently acquired by Medtronic Inc.
In addition to these business roles, Dr. D’Orta continues to serve as a director for the Board of Visitors at Johns Hopkins University and for the Board of Directors to the Georgetown University Hospital. He also holds faculty positions in emergency medicine and surgery at Georgetown, George Washington, and the University of Maryland Schools of Medicine and serves on several administrative and clinical oversight boards.
In the public arena, Dr. D’Orta has held numerous positions in the State of Maryland, including appointment as the State Medical Director. The Governor also named Dr. D’Orta Chairman of the Maryland Commission on Emergency Medical Services, which resulted in comprehensive reforms of the State’s emergency medical system, solidifying Maryland’s reputation as a leader in the field of emergency and trauma care.
Humanitarian relief has played an important role in shaping Dr. D’Orta’s professional life. He played a critical role in coordinating emergency physician teams in response to the earthquakes in Mexico City, Armenia, and San Francisco. He has provided medical aid to the victims of the Amtrak-Contrail disaster in Maryland and led an international medical task force to Kuwait after the Gulf war.
Dr. D’Orta has been honored with numerous awards given by his colleagues in medicine and the community. He has received several special citations from the President of the United States and the Governors and Legislatures of Maryland and Virginia.

Sean O'Donnell
President &
COO
Over the last 10 years, Sean O’Donnell has served in Fortune 500 health care companies leading the development and implementation of consumer health strategies and alliances.
Prior to joining Consumer Health Services, Mr. O’Donnell worked with Medtronic Inc. directing health policy and reimbursement strategies from 1996 to 1998 launching new medical device therapies with Medicare and major third party payers. Until 2000, he led corporate direct-to-consumer marketing programs utilizing integrated online/offline media to promote Medtronic therapies and their associated health system partners. In 2002 he was responsible for developing Medtronic’s co-marketing, distribution and ally development programs in diabetes care in partnership with external diagnostic manufacturers and retail channels. Most recently, Mr. O’Donnell was the global director for Novartis AG in Basel, Switzerland in charge of developing and executing 3rd party alliances with academic partners and national health systems to support Novartis’ pharmaceutical strategies in the development and launch of new medicines in cardiovascular and infectious disease.
Mr. O’Donnell holds a Masters Degree in Health Policy and Management from the Johns Hopkins University School of Public Health and is a graduate of Santa Clara University.

Chris Hoyt
Senior Vice President, Corporate Development and Investor Relations
Mr. Hoyt has over 15 years of finance and corporate development experience in a number of technology industries. He has been directly involved in raising over $400 million in debt and equity capital for emerging companies as well as completing over two dozen acquisitions and divestitures with aggregate consideration in excess of $2 billion.
Prior to joining the Consumer Health Services team, he served as the Vice President, Corporate Development for a public telecommunications company where his responsibilities included the development of the strategic plan and execution of the external growth plan through acquisitions and partnerships. Mr. Hoyt has also held a number of financial and strategy positions in the telecommunications industry including VP, Financial Planning & Analysis for a $175 million satellite communications provider and Director of Corporate Planning for a public DSL provider. Key accomplishments in these roles include; the $200 million restructuring and sale of the satellite services company, the development of the financial model and business planning process for the successful IPO of the DSL provider and leading a $150M equity investment from SBC Communications to enable nationwide expansion of the DSL footprint.
Mr. Hoyt holds a B.S. Degree in International Finance from American University.

Manish N. Oza, MD
Regional Medical Director
As part of our focus to develop a world-class clinical affairs team, an emergency physician by training, Dr. Oza provides an important clinical interface for CHS with its health system partners as well as with the medical practices under its management. Dr. Oza will actively lead efforts for CHS to begin clinical preparations with our partners for the development and launch of retail-practice sites.
In his role, he will also be collaborating more broadly with the medical leadership in our partner health systems throughout the United States to develop and apply consistent quality assurance processes across the retail-based medical practices under our management. He will also coordinate and support the retail-practice medical directors in their efforts to optimize clinical work-flow and ensure an exceptional care experience for all of their patients. Additionally, he will advise the CHS management team on emerging best-practices, services and technology solutions which further enable the practices we manage to remain leaders in the rapidly emerging field of retail-based consumer medicine.
>>  Trevor Arnold
Director of Operations
Trevor Arnold has over ten years experience as an operations and business development specialist with consumer goods and services.
Prior to joining Consumer Health Services, Trevor worked with one of the largest residential real estate companies in Southern California to organize an in-house marketing department, launch a branch office and pilot an electronic referral program.
Mr. Arnold managed the United States operations of Singapore based Maglink International Ltd Pte (Maglink). Maglink developed and marketed data storage media and electronic devices for sale in the United States. Maglink USA coordinated marketing resources in Singapore, website development from India and manufacturing in Singapore/China with financing and sales group in North America.
Trevor Arnold has a Bachelors Degree in Political Science and Philosophy from Dickinson College.
>>  William Church
Controller
William Church joined Consumer Health Services, Inc. in March of 2008 and serves as the Company’s Controller. Mr. Church is responsible for all of the Company’s general accounting, financial reporting, budgeting, billing and collections, purchasing, and corporate compliance. Additionally, he plays an important role in developing strategies to grow the Company, control costs, and enhance profitability.
Prior to joining CHS, Mr. Church served as the Controller and Administrator of a leading Radiation Oncology Center in the Southeast and later as the Controller of a large solid waste hauling division of a Fortune 500 Company. He has over 17 years of experience in accounting, finance, and healthcare management and has a strong background in cost management, procurement of financing, implementing new systems, and developing strategies which bring value to shareholders.
